I grant permission to the rights of my image, likeness and the sound of my voice as recorded in sessions with Joy Musser and Musser Voice LLC using both audio and video recording media. I understand that my voice or image may be edited, copied, published, or distributed and waive the right to inspect the finished product in which my audio or video likeness appears and that I will not receive any rights to royalties or other compensation for the use of my recording or image. I understand that this material may be used in varied settings within an unrestricted geographic area and may be displayed electronically via the Internet, television, and/or print media.
It is contemplated that audio, video, and/or photo recordings may be used for the following purposes, including but not limited to conference presentations, educational presentations and courses, informational presentations, online educational courses, and educational videos. There is no time limit on the validity of this consent and release.
This release applies only to audio, video or photographic recording collected as part of the sessions identified by this document only. I understand that if I refuse to grant permission for the use of audio, video, and photographic recordings other than for my own benefit, it will not stop me from being seen as part of my evaluation, treatment, and follow-up.
By signing this form, I/we acknowledge that I/we have completely read and fully understand the above release and agree to be bound thereby and release any and all claims against any person or organization utilizing this material.
